چکیده

This article portrays the authors’ experience with a complex lower limb bone and soft tissue defect, following chronic osteomyelitis pathological fracture, which was managed by multidisciplinary orthoplastic team. The decision for functional amputation versus salvage deemed necessary, enhanced principles of “spare parts” in reconstructive microsurgery. case describes successful use osteocutaneous distal tibia turn-up fillet flap that allowed ‘lowering level amputation’ from through knee to below-knee preserve joint function. We comprehensibly review reports flaps effectively amputation, also applying ‘spare-parts’ surgery explore how these concepts refine approaches when is not possible enhance robust technique modified BKA reconstructions provides sufficient length achieve tough, sensate stump joint.

Spare Parts Surgery—Salvage of a Below Knee Amputation Stump

“Spare parts surgery” is well-described in the plastic surgery literature. In the setting of trauma, otherwise discarded parts can be utilized for reconstruction resulting in a superior functional and aesthetic outcome for the patient. We describe the use of spare part surgery in order to preserve a functional knee articulation in a patient undergoing amputation for compartment.

A rationale for skew flaps in below-knee amputation surgery.

The use of thermography in the assessment of amputation levels has demonstrated a medial to lateral thermal gradient in many cases. In order to see whether this reflected a true medial to lateral skin blood flow gradient, a prospective study was set up to measure blood flow medially and laterally below the knee. Twenty-one patients, presenting for amputation assessment with end-stage peripheral...

Below-Knee Amputation Surgery

Throughout the history of lower-extremity amputation surgery, the relative emphasis placed on various parts of the procedure has undergone many changes as new techniques have become available and as new goals have been appreciated. At present, the amount and direction of available knowledge demand the surgeon's concern with fundamental principles. Since some of these problems must be considered...

A Case of Nonisland Pedicled Foot Fillet Flap for Below-Knee Amputation Stump Wound: Treatment Option for Compartment Syndrome after Fibular Free Flap Surgery

Despite the frequent use of the fibular free flap, there have been no reports of severe compartment syndrome of the donor leg that necessitated limb amputation. A 66-yr-old man had a fibular osseous free flap transfer from the left leg to the mandible that was complicated by postoperative compartment syndrome. An extensive chronic leg wound resulted, which was treated with multiple debridements...

The Below-knee Amputation "

rhroughout the United States and Canada an estimated 80 percent of all major elective civilian amputations result from ischemia. All but a relatively few involve the lower extremity. Significant improvements in below-knee prostheses and important advances in surgical and postsurgical management now allow amputation below the knee in a majority of patients. In a consecutive series of 96 unselect...
